Register For a New Account

Registration for AMH Online Reports is a two-step process. Complete both steps and the site administrator will activate your account.

Step 1:

  • Download the PDF registration form.
  • Print the form and fill it out with your user information.
  • Have your agency director or another authorized person sign the form.
  • Fax or mail the form to Addiction and Mental Health Division (see instructions on the form).

Step 2:

Provide the requested information below. An asterisk denotes a required field.
